Objective
· prepare a comprehensive client anamnesis and conduct client examination with appropriate tools, methods, and techniques
· order and/or conduct additional examinations and interpret their results to determine the need for treatment
· make evidence-based conclusions about your client's health independently in own area of expertise, plan care/treatment, and send client to treatment (referral) if necessary
· take responsibility of the care of clients with non-communicable diseases in different phases of the treatment path independently by consulting the multiprofessional team
· guide /educate client concerning the benefits of medicines, possible side effects, combined effects, and importance to follow treatment guidelines as part of self-management
· support client´s rehabilitation
· monitor and evaluate health, effects of care and self-management of client with long term condition
· assess and develop client and drug safety at the work unit level
Content
· Comprehensive assessment and clinical examination of the client's health
· Medical and non-medical treatment of the client from the point of view of nursing
· Supporting rehabilitation and support methods for rehabilitation
· Reception of an advanced clinical practitioner
· Development of clients and drug safety
· Health and care plan as a part of the chronic care model
